In episode 60, Akiza Izinski uses this card in her Ground Duel against Misty Tredwell. After Akiza Normal Summons this card, she activates its effect, drawing "Rose Fairy". Akiza Special Summons "Rose Fairy" with its own effect. Akiza then tunes "Rose Fairy" with this card to Synchro Summon "Black Rose Dragon". In the next episode, this card is seen in the Graveyard when Akiza revives "Black Rose Dragon" with "Shining Rebirth".

In episode 75, Akiza used this card in her Turbo Duel against Tetsu Trudge. After she Normal Summons this card, she activates its effect, drawing "Dark Verger". Trudge then activates "Climactic Barricade" to prevent all monsters that are Level 4 or lower from attacking this turn. "Climactic Barricade" will also inflict 500 damage to Akiza for each Level 4 or below monster she controls on her End Phase. Akiza then activates "Speed Spell - Summon Speeder" to Special Summon "Dark Verger" from her hand. She then tunes "Dark Verger" with this card to Synchro Summon "Splendid Rose".

In episode 99, Akiza used this card in her Turbo Duel against Andre. After Akiza Normal Summons this card, she activates its effect, drawing "Rose Fairy". Akiza Special Summons "Rose Fairy" with its own effect. Akiza then activates "Speed Spell - Angel Baton" to draw two cards and send "Glow-Up Bulb" from her hand to the Graveyard. She then tunes "Rose Fairy" with this card to Synchro Summon "Black Rose Dragon".
- In the 4kids dub of this episode, Akiza mistakenly calls this card "Witch of the Black Forest".

In the non-canon duel YCS Providence 2012 Special: Yusei vs. Akiza, Akiza Normal Summons this card and activates its effect, drawing "Lord Poison". "Witch of the Black Rose" then attacks Yusei's "Nitro Synchron", but the attack is negated by Yusei's "Scrap-Iron Scarecrow". The next turn, "Witch of the Black Rose" is destroyed by battle by Yusei's "Junk Warrior".
