"XY-Dragon Cannon":

When dueling Yugi in his battle tower, Kaiba first fuse X-Head Cannon and Y-Dragon Head into XY-Dragon Cannon, and then next turn he fused it with Z-Metal Tank to summon XYZ-Dragon Cannon, which is impossible in the real game. He later tributed this one card alone to summon "Obelisk the Tormentor". Kaiba combined X-Head Cannon and Y-Dragon head, and called the new monster "XY-Dragon Cannon", but it is possible that it wasn't a fusion and he just called the combination "XY-Dragon Cannon" because the hologram projection shows the appearance of XY-Dragon Cannon whenever the two Union monsters combined (which also means it's possible XY-Dragon Cannon doesn't exist in the Anime).
