The Millennium Eye is one of the seven [[Millennium Items (manga)|Millennium Items]], which were created to help Egypt defend from foreign invaders.

[[Akhenaden (manga)|Akhenaden]] became the first owner of the Millennium Eye and had his left eye removed to insert it. It was written in the [[Millennium Spellbook|Millennium Tome]] that the Eye would grant a wish to the person who wore it. Akhenaden thought the notion of a wish was absurd, but regardless wished that his son, [[Seto (manga)|Seto]] become [[Pharaoh]].<ref name="d 308" />

Akhenaden took part of the exorcism of [[Dark Spirit of Chaos (manga)|Dark Spirit of Chaos]], from [[Dark Spirit of Chaos' host (manga)|a criminal]]. He used the Millennium Eye to expose Dark Spirit of Chaos' true form.<ref>{{chapter|Yu-Gi-Oh!|283|ref}}</ref>

[[Bakura, King of Thieves (manga)|Bakura, King of Thieves]] arrived at the [[Pharaoh's palace]], having plundered [[Akhenamkhanen (manga)|Akhenamkhanen]]'s tomb, and intended to take the Priests' Millennium Items. Akhenaden used the Millennium Eye to gaze into Bakura's soul and was surprised to see a bottomless pit of shadow with a powerful hatred hidden inside.<ref>{{chapter|Yu-Gi-Oh!|285|ref}}</ref>

The Priests conducted [[Priests' Diaha|mock ''Diaha'']] to prepare from further attacks from Bakura. In one of them, Akhenaden used the Millennium Eye to summon [[Dark Ushebti]] from a stone slab in the [[Shrine of Wedju (location)|Shrine of Wedju]].<ref>{{chapter|Yu-Gi-Oh!|291|ref}}</ref>

Bakura later snuck into the Shrine of Wedju and confronted Akhenaden. Akhenaden attempted to summon a ''ka'' from a stone slab, but Bakura's Diabound destroyed the slab before the ''ka'' could materialize. Bakura used the power of the [[Millennium Ring (manga)|Millennium Ring]] to fling Akhenaden against a slab.<ref>{{chapter|Yu-Gi-Oh!|298|ref}}</ref> Rather than killing him and taking the Millennium Eye, Bakura figured making Akhenanden his pawn would be more fitting justice for the [[Kul Elna massacre]]. Bakura used the Millennium Ring to infuse part of his evil within the Millennium Eye. The process left Akhenaden unconscious and bleeding from the eye socket. The other Priests mistook Akhenaden's injuries as the result of a failed attempt at stealing the Millennium Eye.<ref>{{chapter|Yu-Gi-Oh!|299|ref}}</ref>

Akhenaden became more merciless since Bakura tampered with the Millennium Eye. While Pharaoh [[Atem (manga)|Atem]] was missing, he told Seto to kill [[Kisara (manga)|Kisara]] to take her [[The White Dragon (manga)|White Dragon]] ''[[ka]]'' and become Pharaoh. Seto was confused by the change in personality, having understood Akhenaden to be merciful even towards sinners. The evil further manifested when he killed a guard who refused to tell him where Kisara was being kept in the palace. (The killing was not present in earlier printings of the manga.)<ref>{{chapter|Yu-Gi-Oh!|312|ref}}</ref>

Akhenaden acquired all the Millennium Items and gouged the Millennium Eye from his eye socket, so that he could place all seven in the [[Millennium Stone|Tablet of the Pharaoh's Memories]], as part of a dark pact with [[Zorc Necrophades (manga)|Zorc Necrophades]] to gain power. He became the High Priest of Darkness<ref>{{chapter|Yu-Gi-Oh!|320|ref}}</ref> and reinserted the Millennium Eye into his socket after completing the pact.<ref>{{chapter|Yu-Gi-Oh!|322|ref}}</ref> He then waged war with Atem.<ref name="d 305">{{chapter|Yu-Gi-Oh!|305|ref}}</ref>

The [[Millennium Items (manga)|Millennium Items]] were looked after by generations of [[Tomb Guardian]]s with the Eye, [[Millennium Ring (manga)|Ring]], [[Millennium Key (manga)|Key]], and [[Millennium Scales (manga)|Scales]] eventually falling into the care of [[Shadi (manga)|Shadi]]. The Tomb Guardians saw it as their mission to find suitable hosts for the Items. They believed that if they did that they would find the other Millennium Items, as the Items are drawn to each other and when that time came the Items would be united in the [[Millennium Stone|Tablet of the Pharaoh's Memories]].<ref name="d 132">{{chapter|Yu-Gi-Oh!|132|ref}}</ref>

Pegasus became interested in the ancient Egyptian beliefs in life after death, following the death of his lover, [[Cyndia (manga)|Cyndia]]. He traveled to Egypt to learn more and found himself observing Tomb Guardians testing [[Millennium Ring thief|a thief]] for worthiness of the [[Millennium Ring (manga)|Millennium Ring]]. Pegasus was caught spying on the affair and brought before Shadi. He begged to be let go and promised not to tell anyone what he had seen. However, he was only permitted to leave if he became the bearer of a Millennium Item, so he was to be tested by the Millennium Eye. If he were deemed worthy, Shadi said he would be granted his dearest wish. The other men cut out Pegasus's left eye and inserted the Millennium Eye in its socket. Pegasus dearest wish to see Cyndia again. He passed the test and for a moment was embraced by an apparition of her.<ref name="d 132" />

===Pegasus and Bandit Keith's Duel===
[[Maximillion J. Pegasus (manga)|Maximillion J. Pegasus]] faced [[Bandit Keith (manga)|Bandit Keith]] in [[Maximillion J. Pegasus and Bandit Keith's Duel (manga)|a televised game]] of ''[[Duel Monsters (manga)|Duel Monsters]]'' in the [[New York Duel Stadium]] and used the opportunity to humiliate Keith. He used the Millennium Eye to read Keith's mind and wrote down some instructions. He handed the instructions to a beginner, [[Tom (manga)|Tom]], and got him to finish the Duel in his place, defeating Keith.<ref>{{chapter|Yu-Gi-Oh!|87|ref}}</ref>

[[Seto Kaiba (manga)|Seto Kaiba]] witnessed Pegasus' mind-reading abilities and incorporated elements that could potentially combat it into the development of his [[Duel Disk]] technology. Players were required to stand at least five meters apart, which he hoped would put them out of range of the Mind Scan<ref>{{chapter|Yu-Gi-Oh!|86|ref}}</ref> and [[Solid Vision]] projections of cards in players' hand obscured the view of their face and body.<ref>{{chapter|Yu-Gi-Oh!|101|ref}}</ref>

[[Maximillion J. Pegasus (manga)|Maximillion J. Pegasus]] played a [[Shadow Game]], where he used [[Pegasus' videotape|a videotape]] to face [[Dark Yugi (manga)|Dark Yugi]] in a game of ''[[Duel Monsters (manga)|Duel Monsters]]'' through the television. Using the Millennium Eye's [[Mind Scan]] power, he read Dark Yugi's mind throughout the Duel. When he won, he used the Eye to inflict the "SOUL TV" [[Penalty Game]], trapping the soul of [[Sugoroku Mutou (manga)|Sugoroku Mutou]] in the videotape.<ref>{{chapter|Yu-Gi-Oh!|62|ref}}</ref>

During the first night of the [[Duelist Kingdom (manga)|Duelist Kingdom]] tournament, [[Ryo Bakura (manga)|Ryo Bakura]] discovered that the Millennium Ring was pointing in the direction of [[Pegasus Castle]] and suspected it was pointing towards the Millennium Eye.<ref>{{chapter|Yu-Gi-Oh!|85|ref}}</ref> The next day, Bakura made use of this information when he and his friends were lost underground, as they were able to use the Ring to know which direction they were traveling.<ref>{{chapter|Yu-Gi-Oh!|92|ref}}</ref>

Pegasus' prisoner, [[Mokuba Kaiba (manga)|Mokuba Kaiba]] made an escape attempt, so Pegasus used the "Mind Card" ability of the Millennium Eye to seal Mokuba's soul in the card, "[[Soul Prison (manga)|Soul Prison]]". This left Mokuba with just enough volition to obey Pegasus' orders.<ref name="d 108">{{chapter|Yu-Gi-Oh!|108|ref}}</ref>

[[Seto Kaiba (manga)|Seto Kaiba]] faced Pegasus in a [[Seto Kaiba and Maximillion J. Pegasus' Duel (manga)|Duel]] and insisted on using the [[Duel Disk]]s he had developed to combat the Millennium Eye's Mind Scan. However Pegasus thought it foolish to fight on his enemy's terms, so would only accept if Mokuba's soulless body acted as his body, holding his Duel Disk. Kaiba refused to do that and they Dueled without the Duel Disks.<ref name="d 108" /> This enabled Pegasus to use the Eye to read Kaiba's mind. <ref>{{chapter|Yu-Gi-Oh!|109|ref}}</ref> When he won, he used the Eye to administer the "Soul Card" Penalty Game on Kaiba, trapping his soul in another "Soul Prison".<ref>{{chapter|Yu-Gi-Oh!|111|ref}}</ref>

Pegasus had dinner served to tournament semifinalists, who were disturbed to discover he had placed [[Replica Millennium Eye (manga)|replica Millennium Eye]]s in their soup. He then announced the [[Heart-Pounding Tournament Entrée Bingo Game]] and instructed them to crack open their replica Eyes to find a letter used to determine the matchups for the semifinals.<ref>{{chapter|Yu-Gi-Oh!|112|ref}}</ref>

During Dark Yugi and [[Mai Kujaku (manga)|Mai Kujaku]]'s [[Dark Yugi and Mai Kujaku's Duel (manga)|semifinal]], Pegasus used the Millennium Eye to read Dark Yugi's mind and confirmed Mai's suspicions that Dark Yugi was focused on Dueling Pegasus and not concentrating on the Duel at-hand.<ref>{{chapter|Yu-Gi-Oh!|115|ref}}</ref>

During [[Katsuya Jonouchi (manga)|Katsuya Jonouchi]] and [[Bandit Keith (manga)|Bandit Keith]]'s [[Katsuya Jonouchi and Bandit Keith's Duelist Kingdom Duel|semifinal]], Pegasus wondered how Jonouchi was doing so well when his experience, strategy, and Deck strength were so far behind those of Keith. He opted to read both their minds with the Millennium Eye; seeing despair and revenge in Keith and hope and trust in Jonouchi.<ref>{{chapter|Yu-Gi-Oh!|121|ref}}</ref>

Keith confronted Pegasus with a knife after losing to Jonouchi. But Pegasus was aware Keith had cheated in the Duel, so he used the Millennium Eye to subject him to the "Hand and Gun" Penalty Game. This caused Keith to believe his hand transformed into a gun and he involuntarily played [[Russian roulette]], killing himself.<ref>{{chapter|Yu-Gi-Oh!|124|ref}}</ref>

Pegasus dominated the beginning of the championship match with Dark Yugi, by using the Millennium Eye to read Dark Yugi's thoughts and appropriately counter his moves.<ref>{{chapter|Yu-Gi-Oh!|126|ref}}</ref> [[Yugi Mutou (manga)|Yugi Mutou]] then proposed a "[[Mind Shuffle]]" strategy where they use the Millennium Puzzle to switch which of them was in control of their body just as Pegasus was about to read their mind. This enabled each Yugi to take a turn setting up a move, then switch to the opposite Yugi, who would be unaware of what had been set up, making him incapable of leaking it when Pegasus read his mind. Dark Yugi taunted Pegasus, saying he had lost his Duelist instinct by relying on the Millennium Eye for so long.<ref>{{chapter|Yu-Gi-Oh!|128|ref}}</ref> Once his "[[Toon World (manga)|Toon World]]" strategy was defeated, Pegasus used the Millennium Eye to make the game a [[Shadow Game]]. The mental endurance required to maintain monsters' ethereal form in the Shadow Game wore Yugi down, leaving just Dark Yugi whose mind Pegasus could read.<ref>{{chapter|Yu-Gi-Oh!|129|ref}}</ref> However at the climax of the Duel, Yugi's friends appeared in Dark Yugi's mind and deflected the power of the Eye.<ref>{{chapter|Yu-Gi-Oh!|130|ref}}</ref>

On losing to Dark Yugi, Pegasus upheld his promise to Dark Yugi and released the souls of Sugoroku Mutou, Seto Kaiba, and Mokuba Kaiba whom he had trapped using the Millennium Eye. He then informed Dark Yugi on how he came to posses the Eye.<ref name="d 132" />

[[Dark Bakura (manga)|Dark Bakura]] approached Pegasus shortly after the championship match and gouged the Millennium Eye from his socket, killing him. He left one of the replica Millennium Eyes from the soup with Pegasus, telling him to take it with him to Hell.<ref>{{chapter|Yu-Gi-Oh!|133|ref}}</ref>

He was unwilling to hand over the [[Millennium Ring (manga)|Millennium Ring]] any sooner than Yugi needed it. Atem placed the Millennium Eye along with the other six Millennium Items into the [[Millennium Stone|Tablet of the Pharaoh's Memories]] in the [[Shrine of the Millennium Stone|Shrine of the Underworld]]. This caused him to split into a separate body from Yugi, so that the two may face each other for the [[Rite of the Duel]].<ref>{{chapter|Yu-Gi-Oh!|338|ref}}</ref> By experiencing defeat, Atem was deemed ready to pass on. His passing through the door to the afterlife, caused the Tablet of the Pharaoh's Memories to break and fall into the depths of the earth, taking the Millennium Items with it.<ref>{{chapter|Yu-Gi-Oh!|343|ref}}</ref>
