"Deck Out" is a type of deck (also called "Mill") and a term. To "deck out" an opponent means to reduce the number of cards in their deck to 0. If they must draw a card when they have 0 cards in their deck, they lose the duel. This is common with Lightsworn decks.
So far, only two actual people in the anime have lost due to decking out; Strings from Yu-Gi-Oh! (second series) and Howard X Miller from Yu-Gi-Oh! GX.
The first three main male protagonists have nearly decked out at least once in their own main series. Yugi Muto nearly Decked Out in his duels against Leon and later against Yami Bakura. Jaden Yuki nearly lost in his duel against Howard X Miller and Sartorius the second time they dueled. Yusei Fudo nearly lost in his duels against Mr. Armstrong and Jean.
