This is a two part question I have.
My first is, when a monster is returned by the effects of Future Visions, is it just placed back, or is it a kind of summon?
My second question is, if I have future visions on the field, and my opponent summons breaker, will breaker get his spell counter before or after future visions effect?

1) The monsters are just returned to the field in face-up Attack Position, they are not re-summoned.
2) If you have Future Visions on the field and your opponent summons Breaker the Magical Warrior, their effects will try to activate at the same time and will form a chain according to SEGOC.
- Chain Link 1: The "When this card is Normal Summoned, place 1 Spell Counter on it." effect of Breaker. (Turn Player's Mandatory Trigger Effect)
- Chain Link 2: The "When a monster is Normal Summoned, remove from play that monster." effect of Future Visions. (Non-Turn Player's Mandatory Effect)
Then you resolve the chain in reverse:
- Chain Link 2: Future Visions will remove from play Breaker.
- Chain Link 1: The effect of Breaker will resolve without effect.
