The Millennium Eye is one of the seven [[Millennium Items (manga)|Millennium Items]], which were created to help Egypt defend from foreign invaders.

Akhenaden took part of the exorcism of [[Dark Spirit of Chaos (manga)|Dark Spirit of Chaos]], from [[Dark Spirit of Chaos' host (manga)|a criminal]]. He used the Millennium Eye to expose Dark Spirit of Chaos' true form.<ref>{{chapter|Yu-Gi-Oh!|name="d 283|ref}}<" /ref>
[[Bakura, King of Thieves (manga)|Bakura, King of Thieves]] arrived at the [[Pharaoh's palace]], having plundered [[Akhenamkhanen (manga)|Akhenamkhanen]]'s tomb, and intended to take the Priests' Millennium Items. The [[Millennium Items (manga)|Millennium Items]] were looked after by generations of [[Tomb Guardian]]s with the Eye, [[Millennium Ring (manga)|Ring]], [[Millennium Key (manga)|Key]], and [[Millennium Scales (manga)|Scales]] eventually falling into the care of [[Shadi (manga)|Shadi]]. The Tomb Guardians saw it as their mission to find suitable hosts for the Items. They believed that if they did that they would find the other Millennium Items, as the Items are drawn to each other and when that time came the Items would be united in the [[Millennium Stone|Tablet of the Pharaoh's Memories]].<ref name="d 132">{{chapter|Yu-Gi-Oh!|132|ref}}</ref>
Pegasus became interested in the ancient Egyptian beliefs in life after death, following the death of his lover, [[Cyndia (manga)|Cyndia]]. He traveled to Egypt to learn more and found himself observing Tomb Guardians testing [[Millennium Ring thief|a thief]] for worthiness of the [[Millennium Ring (manga)|Millennium Ring]]. Pegasus was caught spying on the affair and brought before Shadi. He begged to be let go and promised not to tell anyone what he had seen. However, he was only permitted to leave if he became the bearer of a Millennium Item, so he was to be tested by the Millennium Eye. If he were deemed worthy, Shadi said he would be granted his dearest wish. The other men cut out Pegasus's left eye and inserted the Millennium Eye in its socket. Pegasus dearest wish to see Cyndia again. He passed the test and for a moment was embraced by an apparition of her.<ref name="d 132" />

Pegasus]] played a [[Shadow Game]], where he used [[Pegasus' videotape|a videotape]] to face [[Dark Yugi (manga)|Dark Yugi]] in a game of ''[[Duel Monsters (manga)|Duel Monsters]]'' through the television. Using the Millennium Eye's [[Mind Scan]] power, he read Dark Yugi's mind throughout the Duel. When he won, he used the Eye to inflict the "SOUL TV" [[Penalty Game]], trapping the soul of [[Sugoroku Mutou (manga)|Sugoroku Mutou]] in the videotape.<ref>{{chapter|Yu-Gi-Oh!|name="d 62|ref}}<" /ref>
During the first night of the [[Duelist Kingdom]] tournament, [[Ryo Bakura (manga)|Ryo Bakura]] discovered that the Millennium Ring was pointing in the direction of [[Pegasus Castle]] and suspected it was pointing towards the Millennium Eye.<ref>{{chapter|Yu-Gi-Oh!|85|ref}}</ref> The next day, Bakura made use of this information when he and his friends were lost underground, as they were able to use the Ring to know which direction they were traveling.<ref>{{chapter|Yu-Gi-Oh!|92|ref}}</ref>
