1. Not all counters, only all spell counters are removed.
 
 
4. you can it activate in response to an attack declaration. You can activate it if ryko was flipp summoned. You can't activate it if ryko was flipped face up by an attack because when its effect activates it is still the damage step. However you can activate it after the damage step is over, see the link in the above answer for more information about that.
