* In episode 113, this card was used by the Big Five during their Duel against Yami Yugi and Joey. In this episode, this card was a Ritual Monster, not a Fusion Monster, with its corresponding Ritual Spell Card being "Dragon Revival Ritual". It required a Tribute of five monsters of different attributes in order to Ritual Summon it, though this requirement goes unmentioned in the English version. The Big Five used their own Deck Masters as the Tributes for "Dragon Revival Ritual": the WATER-attribute "Deepsea Warrior", the WIND-attribute "Nightmare Penguin", the EARTH-attribute "Judge Man", the FIRE-attribute "Robotic Knight" and the DARK-attribute "Jinzo" ("Nightmare Penguin" is a WATER-attribute monster in the TCG/OCG). When it was Summoned, it destroyed all cards on Yugi and Joey's side of the field, but it could not attack the turn it was Summoned. Yugi used "Polymerization" to fuse his Deck Master with Joey's so that he could Fusion Summon "Dark Flare Knight" which became the new Deck Master for Yugi and Joey. This card was then attacked by "Dark Flare Knight". Since the latter was weaker, it was destroyed by the counterattack made by this card. Yugi then used the effect of "Dark Flare Knight" to reduce the Battle Damage he would take to 0 and Special Summon "Mirage Knight" which became the new Deck Master for Yugi and Joey. This card was then destroyed by "Mirage Knight", but the Big Five activated "A Deal with Dark Ruler" to negate the Battle Damage they would receive and Special Summon "Berserk Dragon" in this card's place. "Berserk Dragon" then became the new Deck Master for the Big Five.

Yu-Gi-Oh!

• This card is also known by the Japanese name F.G.D., commonly rendered in the anime and manga as Five God Dragon.
• In episode 45, this monster was known as the Mythic Dragon and was under the control of The Big Five. This monster was to be summoned after Kaiba was going to be sacrificed, but that didn't happen. The Big Five then rewrote the program to summon this monster immediately to the field. It possessed the same effect as the Fusion version, preventing it from being destroyed by battle except by a LIGHT-attribute monster. "Curse of Dragon", "Blue-Eyes White Dragon", "Red-Eyes B. Dragon", and "Harpie's Pet Dragon" all attack this monster, but four of this monster's heads then nullified the attacks. This monster then destroyed Joey's "Red-Eyes B. Dragon" which caused Joey to disappear as his Life Points then dropped to 0. Mai then tries to avenge Joey by attacking with "Harpie's Pet Dragon", but this monster then destroys it. Mai then disappears as her Life Points then dropped to 0. This monster then attacked Kaiba, but Mokuba pushes him out of the way and takes the hit instead, causing him to disappear. Yami Yugi and Seto Kaiba then fuse their "Black Luster Soldier" and "Blue-Eyes Ultimate Dragon" into the 5000 ATK power LIGHT-attribute "Dragon Master Knight". The latter then gets powered up by its own effect, making its ATK 9000. "Dragon Master Knight" then attacks and destroys this monster, defeating it once and for all.
• In episode 98, this monster appears in a flashback Kaiba has when he recognizes The Big Five as the same people who trapped him in his own virtual reality game.
• In episode 113, this card was used by the Big Five during their Duel against Yami Yugi and Joey.
• This card also appears in a flashback Joey has when he remembers facing "Five-Headed Dragon" previously and this monster being destroyed by "Five-Headed Dragon" during the battle which caused him to be deleted until the Princess revived him.
• This card also appears in a flashback Leichter has when he remembers Yugi using this card to defeat "Five-Headed Dragon" as the final move in the battle between Yugi and his allies and the Big Five after Yugi states that they defeated "Five-Headed Dragon" before. Leichter then claims that even though this card was able to defeat "Five-Headed Dragon" last time, Yugi can't do the same thing again as he doesn't have the proper cards necessary to summon it.
• In episode 185, the Fusion version of this card makes its first actual appearance, being used by the KC DuelTek 760 against a Kaiba Land patron. The KC DuelTek 760 activated "DNA Surgery", turning its "Battle Ox", four "Sheep Tokens" and the patron's "Gagagigo" into Dragon-Type monsters. The Dueltek then fused its own monsters with "Polymerization" to summon this card. The Dueltek then had this card attack and destroy "Gagagigo", causing the patron to lose the duel. This marked the first and only time to date that a Five-Headed Dragon was actually able to defeat the player facing it.

Yu-Gi-Oh! GX

• In episode 5, this is the card that Professor Banner picks up for his scary story when he decided to play along with Jaden and his friends while they were telling scary stories. He explains to Jaden, Syrus, and Chumley a story about the Abandoned Dorm which Jaden gets interested in.
• In episode 160, Trueman Summons two copies of this card against Jaden Yuki. He uses "Future Fusion" to send "Des Volstgalph", "Spirit Ryu", "Rare Metal Dragon", "Infernal Dragon" and "Spear Dragon" from his Deck to the Graveyard and Fusion Summon this card on his Second Standby Phase after the activation of "Future Fusion". Jaden notes that he will win before then, but Trueman then activates "Dragon's Mirror" to immediately Fusion Summon this card on his first turn. Jaden then attacks this card with "Neo-Spacian Grand Mole", intending to return this card back to Trueman's Extra Deck, but Trueman activates "Skill Drain", negating "Grand Mole's" effect. Jaden then plays "Battle by 2" to double "Grand Mole"'s ATK. "Grand Mole" is destroyed by the backfired attack, but thanks to "Battle by 2"'s effect, Jaden was able to survive the attack. Trueman then attacks directly with this card on his next turn, but Jaden activates "Negate Attack" to nullify the attack. Jaden then builds a wall of Defense Position monsters to defend against this card. On Trueman's Standby Phase, "Future Fusion" brought out another copy of this card. He then Tributes one copy of this card with "Thousand Punisher" to make the other copy's ATK 1000 until the End Phase and allow the other copy to attack for every 1000 ATK the Tributed copy had. The Tributed copy had 5000 ATK, so the copy on the field gained five attacks. This card then destroys "Elemental HERO Burstinatrix", "Gallis the Star Beast", "Neo-Spacian Air Hummingbird", "Neo-Spacian Glow Moss", and "Neo-Spacian Dark Panther". Trueman then sets "Dragon's Rage" and "The Dragon's Bead" and intended to use the former to give this card the ablility to do Piercing Damage on his next turn and insure him victory. He also intended to use "The Dragon's Bead" on his next turn to insure that Jaden cannot use Traps to target this card at the cost of discarding 1 card. Jaden then attacks this card with "Elemental HERO Neos". He then discards "Honest" to add the ATK of this card to the ATK of "Neos". "Neos" then destroys this card, winning Jaden the duel.
• In episode 178, this card is seen in a flashback relating back to Jaden's Duel with Trueman in episode 160.
