- This card was known as "Reborn the Monster" in the first 27 episodes of the dub.
- In episode 10, Yami Yugi plays this card against Ghost Kaiba to Special Summon one of the two "Blue-Eyes White Dragons" that were in Ghost Kaiba's Graveyard.
- In episode 13, Yami Yugi plays this card against Yami Bakura. He used to bring back "Cyber Commander", which contained Tristan Taylor's soul at the time. He later returned the card to his hand with the effect of "Magician of Faith" (that contained Téa Gardner's soul at the time). He then used this card to revive "Flame Swordsman", which contained Joey Wheeler's soul at the time.
- In episode 21, Yami Yugi played this card during his Labyrinth Duel against the Paradox Brothers while teaming with Joey. He uses it to revive Dox's "Suijin".
- In episode 24, Seto Kaiba plays this card against Yami Yugi, reviving his "Blue-Eyes White Dragon" as a third head on his "Blue-Eyes Ultimate Dragon"(who had its ATK reduced down to 900 thanks to the combo involving "Mammoth Graveyard", "Polymerization" and "Living Arrow").
- In episode 27, Kaiba plays this card against Maximillion Pegasus, reviving "Saggi the Dark Clown". This was the only move Kaiba could make due to "Crush Card Virus" obliterating all of the cards in his Deck except this card.

This activates the effect of "Card of Safe Return", allowing Strings to draw three cards. This also activated the second mouth effect of "Slifer", reducing the ATK of "Buster Blader" by 2000.
- In episode 70, this card was shown in Kaiba's hand during his Tag Duel with Yami Yugi against Lumis and Umbra. In the next episode, Kaiba uses this card to revive his "Blue-Eyes White Dragon".
- In episode 83, Yami Yugi plays this card against Yami Bakura to revive Bakura's "Dark Necrofear", thus canceling out Bakura's "Dark Sanctuary".

- In episode 132, Yami Yugi plays this card against Kaiba, reviving "Dark Magician". Seto later targets this card with "Magical Trick Mirror", letting him revive "Obelisk the Tormentor" with its effect. Two episodes later, Kaiba plays this card to revive "Blue-Eyes White Dragon". Yugi later uses "Double Spell" to use the effect of Kaiba's copy to revive "Buster Blader".
- In episode 136, Joey plays this card against Kaiba, reviving Kaiba's "Blue-Eyes"(who Joey destroyed via "Enemy Controller" which Joey used via "Graverobber"). Kaiba later attacks that "Blue-Eyes" with another "Blue-Eyes", destroying both monsters in a Double KO. Kaiba then plays his own copy of this card to revive one of the "Blue-Eyes" that was destroyed.
- In episode 138, Yami Marik uses this card in his Duel against Yami Yugi. He adds it to his hand via "Left Arm Offering". Yugi uses "Exchange" to take the card from Yami Marik. Yami Yugi then uses this card to revive "Queen's Knight". When Yami Yugi attacks Yami Marik directly with "Slifer the Sky Dragon", Yami Marik uses "Dark Spell Regeneration" to activate it from the Graveyard and revive "Ra". In the next episode, Marik uses "Zombie's Jewel" to put this card back in his hand. He then uses this card to revive "Ra" again. In the next episode, Marik adds this card to his hand again via "Mining for Magical Stones". In the next episode, Marik uses this card to revive "Ra" again. When Yami Marik tries to use the "Egyptian God Phoenix" effect of "Ra" to destroy "Obelisk", Yugi activates his own copy to revive "Slifer the Sky Dragon" and use it to intercept the effect, destroying it, but saving "Obelisk".
- In the first above-mentioned episode, this card appears in a flashback Joey has when he remembers his Duel with Yami Marik and when he is explaining to his friends that Yami Marik just needs this card in order to bring "The Winged Dragon of Ra" out because many of its devastating effects can be activated when it's revived.
Waking the Dragons
- In episode 145, Gurimo plays this card against Yami Yugi, reviving "Marauding Captain". In the the next episode, Yugi plays his own copy to revive "Valkyrion the Magna Warrior".
- In episode 148, this card appears in Yugi's thoughts when Yami Yugi notes that although Pegasus no longer has the Millennium Eye, he was the chosen one and he was the creator of Duel Monsters.

KC Grand Championship
- In episode 195, Kaiba plays this card against Zigfried von Schroeder, reviving "Blue-Eyes White Dragon".
- In episode 198, Leon von Schroeder plays this card against Yami Yugi, reviving "Cinderella. Yugi uses his own copy to revive "Dark Magician".
Dawn of the Duel
- In episode 200, Kaiba plays this card against Yami Bakura, reviving "Blue-Eyes White Dragon".
- This card also appears within Kaiba's briefcase when he opens it up and takes his Duel Disk with him.
- In episode 224, Yugi Muto and Yami Yugi both has this card in their Cermonial Battle against each other. Yugi seals his copy card with "Gold Sarcophagus". When Yami Yugi uses his own copy to attempt to revive "Slifer the Sky Dragon", it is negated. Ishizu Ishtar comments that the card represents Yugi's final message to Yami: "The dead should not linger in the world of the living".
Yu-Gi-Oh! The Movie: Pyramid of Light
- During the Duel between Pegasus and Kaiba, Maximillion Pegasus forces Kaiba to discard this card via the effect of "Toon Gemini Elf". Pegasus later uses his own copy to revive "Toon Gemini Elf".
- Yami Yugi would later play this card in his Duel against Kaiba. He uses it to revive "Dark Magician Girl". Kaiba later plays his own copy to revive "Blue-Eyes Ultimate Dragon". Yugi later uses "Double Spell" to use Kaiba's copy of this card and revive "Blue-Eyes Shining Dragon".
Yu-Gi-Oh! GX
- In episode 1, this card was seen in Jaden Yuki's hand during his Duel with Dr. Crowler. Dr. Crowler sends this card from Jaden's hand to the Graveyard via the effect of "Confiscation".
- In episode 2, Jaden Yuki tries to activate this card during his Duel against Chazz Princeton, but he never got the chance to do so. He had intended to use this card to revive "Elemental Hero Flame Wingman" which he was going to use to attack and destroy Chazz's "Mefist the Infernal General" and win the Duel, however the Duel ends prematurely due to external influences. Jaden later revealed to Alexis that this card was the one he drew on the last turn of the Duel.
- In episode 3, Jaden uses this card in his Duel against Alexis Rhodes. He uses this card to Special Summon revive "Elemental Hero Sparkman".
- In episode 4, Syrus Truesdale is shown wearing three copies of this card on his headband while he was praying to "Slifer the Sky Dragon" to do well in a test. Another copy of this card was shown on the table that Syrus was kneeling in front of.
- In episode 8, Zane Truesdale plays this card against Jaden. He uses this card to revive "Cyber Dragon".
- In episode 10, Para uses this card during his Tag-Team Duel with Dox against Jaden and Syrus. He revives "Jirai Gumo".
- In episode 20, this card appears when Blair Flannigan drops Zane's Deck.
- In episode 21, this card can be seen when Jaden rebuilds his Deck.
- In episode 23, this card can be seen when Jaden rebuilds his Deck.
- In episode 66, this card appears in a flashback Jaden has when he sees Wheeler as Zane which makes Jaden remember his first Duel against Zane.
- In episode 85, Franz draws this card during his Duel against Jaden. It is drawn via the effect of Jaden's "Neo-Spacian Glow Moss". After "The Winged Dragon of Ra" was destroyed by "Glow Moss", Franz intended to play this card on his next turn to Special Summon "Ra" from the Graveyard, but he is defeated before he can do so.
- In episode 156, this card appears in a flashback to episode 2.

- In episode 1, this card is shown as part of Yuma Tsukumo's Deck when he gathers it up from his desk at the start of the episode.

- In episode 7, two copies of this card appear as part of Yuma's Deck when he drops it.
